The first time you come in for your adjustment, I’ll ask you to fill out just a couple of things. I need to know a little about you! There’s also the legal stuff for you give me permission for the adjustment, and to inform you about how I would share your health information with other doctors. You need to acknowledge how that works by autographing it. Yup, every other doctors office has you sign all of these. Feel free to print these 3 forms and fill them in so that you have it when you first come in.
